What can a PATIENT CENTERED MEDICAL HOME do for me?

• Provide you with better care because your healthcare team knows you and your health history.
• Guide you through the complex healthcare system to help you get the care you need either from us or others.
• Offer you better access to care that is coordinated between your healthcare providers, hospitals, and specialists.
• LCHC follows evidence based protocols when providing treatment and ordering proper diagnostic procedures for medical conditions.

Your health care team will partner with you to help you live in a healthy manner continuously, rather than only reacting to illness and/or injury. As your medical home, LCHC increases your access to care and coordinates your care with   others, as needed.

YOUR ROLE AS A PATIENT in LCHC’s Medical Home

• Be an active participant with your healthcare team.
• Write down your concerns and questions and bring them to your appointment so we can discuss them.
• Provide us with a list of your medications including over-the-counter so we can help prevent interactions.
• Call us before you go to an Emergency Room or hospital, or at least let us know as soon as possible that you have gone there.
• When you go to other health care providers or facilities make sure to tell them you are a patient of LCHC and authorize them to share the information from the visit.

LCHC’s ROLE AS YOUR HEALTHCARE TEAM in the  LCHC Medical Home

• Provide a safe environment to talk about confidential concerns
• Offer responses to ALL your questions and concerns at each appointment
• Work with you in partnership, using shared decision-making, to manage your health
• Serves as the quarterback of your healthcare team, coordinating care both inside and outside the practice
• Inform you of the services offered within our practice

You don’t have to go to the ER unless you really need it.

LCHC Medical Home

For common illnesses, when you or your family member looks or acts sick:

• Fever, Flu, Sore Throat, Cough

For problems that need care now:

• Infections, Asthma, Vomiting
• Injuries, Sprains, and cuts that need stitches
• 24 hour on-call provider

And, as always, for:

• Annual Physicals and immunizations (as needed for wellness and prevention)
• Help managing chronic illnesses or conditions
• For assessment of need to go to the Emergency Room

Emergency Room

In an urgent situation please let us know if you are going to the ER as soon as possible, so we can facilitate your ER care.

Examples:

• When bleeding won’t stop
• When breathing is very difficult
• After serious accident
• After a convulsion that lasts more than 3 minutes
