Welcome to Lāna`i Community Health Center

The Lāna'i Community Health Center’s mission is to take care of the community of Lāna`i by directly providing health services (primary care, dental, behavioral health and selected specialty services) to the community; providing activities and services through partnerships with local organizations as well as the many off-island organizations reaching out to Lāna`i by serving as coordinator, advocate, resource, initiator, and convener;
and working collaboratively to provide space for partners who can provide needed services for Lāna`i.

Appointment Guidelines

To ensure a smooth and efficient experience at our health center, please take note of the following guidelines when scheduling and attending your appointments:

Scheduling Appointments

  • To Schedule an Appointment: Call our office at 808-565-6919 and follow the prompts to book your visit.
  • Same-Day Appointments: If you require urgent care that is not life-threatening, please contact us for a same-day appointment. For life-threatening symptoms, immediately call 911 or proceed to the nearest emergency room.

Before Your Appointment

  • Arrival Time: Arrive at least 15 minutes early to your appointment for check-in and to update any necessary information.
  • Pre-Visit Preparation: Inform your provider of any recent visits to other doctors, emergency rooms, or hospitals since your last appointment. Bring all medications, including prescription, herbal, and over-the-counter, that you are currently taking. Prepare a list of questions or concerns you wish to discuss with your care team.

What to Bring

  • Identification: Always bring a valid photo ID and your insurance card to every appointment.

If You Are Running Late

  • Late Arrivals: We strive to see all patients promptly. If you are late, you may face one of the following:
    • A wait time to be seen.
    • A request to reschedule for a later time the same day or another day.
    • An option to see another available provider on the same day.
    • If no appointments are open, you may choose to wait in case an appointment becomes available, but this cannot be guaranteed.

Cancelling or Rescheduling

  • Advance Notice: If you cannot make your scheduled appointment, please call us at least 24 hours in advance to cancel or reschedule. This allows us to offer the slot to another patient in need of care.
  • Leaving a Message: If you cannot reach someone directly, leave a message with your name, phone number, appointment date and time, and whether you are cancelling or rescheduling, along with the reason.

No Show Policy

  • Missed Appointments: Patients who miss three (3) appointments will be placed on a “Same Day Only” status.

Accessibility

  • ADA Compliance: Our facility is wheelchair accessible.
  • Interpreter Services: If you require an interpreter for your visit, please inform us in advance.
