The Lāna'i Community Health Center’s mission is to take care of the community of Lāna`i by directly providing health services (primary care, dental, behavioral health and selected specialty services) to the community; providing activities and services through partnerships with local organizations as well as the many off-island organizations reaching out to Lāna`i by serving as coordinator, advocate, resource, initiator, and convener;
and working collaboratively to provide space for partners who can provide needed services for Lāna`i.

Facilities Housekeeper

Duties Include But Not Limited To:

  1. Responsible for cleanliness, safety and maintenance of all LCHC facilities, interior and exterior, rented or owned.
  2. Cleaning including but not limited to dusting, sanitizing,washing windows, sweeping, mopping, laundry, emptying trash, and simple gardening maintenance.

Job Requirements

Patient Account Representative

Duties Include But Not Limited To:

  1. Monitor patient account details.
  2. Follow up with insurance claims and denials.
  3. Collect and process payments.
  4. Prepare, review, and send out patient statements.

Community Coordinator

Job Overview: The Community Coordinator provides content leadership, implementation and operational coordination to LCHC projects and initiatives. LCHC seeks an experienced community driven individual for current and future public health programs at the LCHC.   Depending on funding, programs may include physical activity and nutrition, tobacco prevention and control, chronic disease prevention or other public health related areas.

This position will work with the Medical Director, Associate Medical Director and the Executive Director to implement and monitor programs, prepare program reports, as well as participate with evaluation activities to ensure the sustainability of current programs and advance the organization’s public health and wellness goals.  This position will also be responsible to initiate community building efforts and develop and maintain an active community coalition within a geographic or program area in support of public health and wellness goals. This position will work on policy, systems, and environmental changes to create sustainable changes around public health and wellness.

This coordinator will also work with community members, LCHC staff, stakeholders, partners and contractors to envision, develop, coordinate, and implement related meetings and events as needed.   The coordinator may also be responsible for delivery of technical assistance and trainings, coordinate and facilitate meetings with appropriate stakeholders, conduct research on relevant topics, craft and disseminate communications to different audiences, and document and maintain records of the program’s progress and accomplishments.

This position will be responsible to uphold the mission, vision, and values of LCHC, as well as instilling the mission, vision and values in each employee.

Duties:

  1. Supports LCHC leadership in development of programs and activities that focus on the improvement of health outcomes for LCHC patients and the general Lāna’i community. General goals of programs and activities are to address health equity and eliminate health disparities with ideal focus on our vulnerable populations through strategies such as advocacy, programmatic efforts, and/or capacity building.
  2. Develops and implements LCHC initiative projects. Demonstrates leadership in broad outreach and provides ongoing training and technical assistance in the health and wellness in support of LCHC programs.
  3. Initiates community building; provides resources and presentations to stakeholders on different health topics; provides LCHC patients and programs  and LCHC partners with technical support to ensure the continued expansion, diversity and effectiveness of these groups. Coordinates efforts with LCHC staff, including but not limited to wellness coaches, providers, community health workers and outreach workers.
  4. Identify, develop and maintains strategic partnerships that will support further growth and sustainability of the LCHC and its programs.
  5. Work with LCHC leadership to develop short and long‐term goals; provide assistasnce and support in updating LCHC’s strategic plan. Collaborates with appropriate stakeholders, partners, agencies, and organizations on projects, task forces, and committees to accomplish strategic plan goals.
  6. Under direction of LCHC leadership, enlists and develops support from members (staff and volunteers) of the community, government, private and non‐profit agencies to build support and advance program effectiveness. Works collaboratively with this network to ensure implementation of program is effective and successful.
  7. Plans and coordinates LCHC meetings and events in support of LCHC programs, activities and grants.
  8. Promotes sustainability to our health education and grant programs. Researches and develops resources, develops health education ciriculum as approved by the Medical/Associate Medical Director and encourages in‐kind contributions; assists with preparation of grant applications and associated reports; identifies new sources of funding and supports fund raising efforts.
  9. Participates in directing the teaching opportunities for the health education program with Lanai High and Elementary School; stays current in advancements in the health and wellness field, and LCHC programs. Be able to provide analysis on the impact of LCHC programs, activities and initiatives.
  10.  Actively builds grassroots networks. Works with LCHC leadership and staff to develop communication and social media plans.
  11. Working in conjunction with LCHC leadership, supports communication activities to different audiences as required or needed either written or verbal communication or through presentations and through social media.
  12. Manages program goals and objectives and evaluates efforts for effectiveness and areas for improvement.  Participates with related evaluation strategies as required by the programs or initiatives.
  13. Follows all policies and procedures, including HIPAA; promotes and support organization’s mission/vision, goals and values.
  14. Other projects and tasks as assigned.

General Fundamentals

  1. Practices and espouses the LCHC patient care philosophy: All Inclusive culturally sensitive Patient Centered Medical Home care integrated with behavioral health, outreach, and case management.
  2. Ensures compliance with Lāna’i Community Health Center policy and procedures as outlined in the Lāna’i Community Health Center Employee Handbook and Policies posted on the CompanyWeb.
  3. Keeps informed of and complies with all occupational safety guidelines. Takes all reasonable precautions to prevent accidental injury on the job.
