The Lāna'i Community Health Center’s mission is to take care of the community of Lāna`i by directly providing health services (primary care, dental, behavioral health and selected specialty services) to the community; providing activities and services through partnerships with local organizations as well as the many off-island organizations reaching out to Lāna`i by serving as coordinator, advocate, resource, initiator, and convener;
and working collaboratively to provide space for partners who can provide needed services for Lāna`i.

Outreach and Education

Outreach and EducationLāna‘i Community Health Center provides a wide range of services to the residents of Lāna’i, including Outreach and Education. The Outreach and Education Division is dedicated to assisting and educating patients in regard to various care and coverage resources, access to healthcare, and public benefit  programs offered by the State’s Department of Human Services.

Our Outreach and Education Division is a free and voluntary service that serves as an information hub for our patients, community residents, and our entire island.

Our Outreach and Education Division works closely with our Medical and Behavioral Health providers, and Case Management, to provide many services including transportation to and from LCHC appointments, and oral and written translation services to help patients understand and improve their overall health, in mind, body, and spirit.

Our LCHC Outreach and Education staff reach out to the community to share information and advise about LCHC programs and services by providing home visits and participating in community health fairs, events and Saturday’s Open Market.

An Important Part of Your Care

 Lāna‘i Community Health Center is the only organization in our community that provides Health Literacy Classes to patients and community members. Our Outreach and Education Division works with our Medical Director to implement peer educator programs for chronic illness aimed at helping patients understand their  illness and reduce symptoms associated with medical treatment and medications. Our Outreach and Education staff is part of the team focused on your care. As a patient centered health center, we are aware of our patients cultural needs and incorporate those needs into their care. LCHC serves individuals of all ages, ethnicity, gender, and residency—old timers, part-timers, and newcomers.

In what ways can the Outreach and Education Division assist me?

  • Enrollment in DHS public benefits programs (Medical Assistance, SNAP and Financial Assistance, TANF, Child Care, Housing programs)
  • Understanding and navigating health insurance programs to find coverage that best fits the needs of you and your family
  • Translation services for interviews and meetings with the Legal Aid Society of Hawaii and Lāna’i branch of State’s Department of Human Services
  • Transportation to and translation services at health care appointments
  • Understanding family health issues and history
  • Peer educator services
  • Resource and agency referrals
