Lāna‘i Community Health Center provides a wide range of services to the residents of Lāna’i, including Outreach and Education. The Outreach and Education Division is dedicated to assisting and educating patients in regard to various care and coverage resources, access to healthcare, and public benefit programs offered by the State’s Department of Human Services.
Our Outreach and Education Division is a free and voluntary service that serves as an information hub for our patients, community residents, and our entire island.
Our Outreach and Education Division works closely with our Medical and Behavioral Health providers, and Case Management, to provide many services including transportation to and from LCHC appointments, and oral and written translation services to help patients understand and improve their overall health, in mind, body, and spirit.
Our LCHC Outreach and Education staff reach out to the community to share information and advise about LCHC programs and services by providing home visits and participating in community health fairs, events and Saturday’s Open Market.
